Milford Integrated Care Hub provides an interdisciplinary diagnostic process to determine the medical, psychological and functional capability of someone who is living with frailty. Led by geriatricians, it offers outpatient appointments to do full comprehensive assessments of patients.
Service details
We are a team of doctors and nurses, therapists (occupational therapists and physiotherapists) and health care assistants who offer holistic assessments to older people living in Guildford and Waverley. These assessments are called Comprehensive Geriatric Assessments (CGA).
We offer clinic appointments for holistic multi-disciplinary assessment of health and medical needs, so that we can work with individuals and carers to help you with being independent at home and managing health care conditions.
We very much value feedback from all of our patients, and may ask you to complete a satisfaction questionnaire after your appointment.
Who is eligible for the service?
We primarily care for older people in the Guildford and Waverley community, following a referral from your GP or hospital consultant.
The main function of our service is to diagnose any conditions that you have and to treat you, and in turn refer you back to your GP and to other community services for on-going management of your condition.
