Care Home Matron Service

We work in partnership with Procare Community Services (the local GP Federation for Guildford and Waverley) to provide adult community nursing services.

Our Care Home Matrons work collaboratively to provide joined up care for Care Home Residents across Guildford and Waverley, in line with the Enhanced Health in Care Homes National Framework which lays out a clear vision for working with care homes to provide joined up primary, community and secondary, social care to residents of care and nursing homes.

The aim of the service is to:

• Reduce unnecessary hospital admissions and A&E attendance.
• Reduce falls and trauma injury to this cohort of patients.
• Provision of best practice and education update sessions where gaps are identified.
• Increase in End of Life Care and Preferred Place of Care by assisting and identifying an End of Life Care Plan.
