Community Nursing Service

The Community Nursing team visit people in their own home to provide nursing care, support and treatment when their health or wellbeing prevents them from attending a GP surgery or a clinic. 

Procare Community Services (the local GP Federation for Guildford and Waverley) and Royal Surrey NHS Foundation Trust are working in partnership to provide adult community nursing services.

Service details

Each team is led by a district nurse with a specialist qualification who manages a team of registered nurses, associate practitioners and healthcare assistants.

We work closely with GPs, social care, local hospitals, hospices, mental health services, social prescribing, specialist nurses and therapists to help individuals to manage their own conditions and to be as independent as possible, remaining at home wherever possible.

Who is eligible for the service?

The service is available for adults aged over 18 who are registered with a Guildford & Waverley GP and who are unable to attend a GP surgery or clinic due to their current health or wellbeing.

 

This includes patients living in their own homes or residential homes but not nursing care homes.

How to access the service?

Referral to the service can be made by your GP, out-of-hours doctor, acute hospital, local hospice or another health/social care professional. 

We will contact you to assess your needs and either arrange a visit to provide care or signpost you to the right service.
