Booking your birth with us
So that we can make your first appointment to meet us, please complete the self-referral form providing as much information as possible. Questions marked with an asterisk are mandatory.
Details of your midwife booking appointment will be emailed to you within seven working days. Please do not contact the booking coordinator unless you have not heard from us in seven working days. We cannot provide access to consultants or clinics until a midwife booking appointment has been completed.
If you have difficulties completing this form, or have not received your midwife booking appointment, contact the booking coordinator on 01483 464004.
Please note: We have recently seen a significant rise in the number of women wishing to transfer their care to us from other maternity providers. To ensure we are able to continue to meet the mother to midwife ratio for labour care and continue to meet the expected standards for safety in maternity care, we have had to make the decision to limit out of area bookings/transfers of care once we reach our capacity of expected births.
If you live outside of our geographical catchment area for Guildford and Waverley, when you submit your booking form you will be notified by our team if we are unable to accept your referral and advised to contact your local maternity provider. If we accept your referral for pregnancy care, that is only complete once you have had your booking appointment with our team.

If you choose to have your baby with us but live outside the local area, you will receive their antenatal care from the Surrey Blossoms team. These appointments will be based at the hospital in antenatal clinic. Please be aware that once you have had your baby you will be referred back to your local hospital for all your postnatal care. We unfortunately do not hold postnatal clinics for out of area patients. If you have any questions please do not hesitate to discuss with your midwife.
