Admiral Nurse Service – Dementia Specialists

The Admiral Nurse team is part of the wider community nursing service in Guildford and Waverley working with families living at home with dementia.

The service focuses on the family as a whole, making sure carers have the support and skills they need. It also works alongside other community services and can help coordinate the different services that may be involved in supporting the person with dementia.

Service details

The service provides specialist advice, following a personalised assessment of each family and then tailored support with all aspects of dementia, from dealing with symptoms like distressed behaviour and sleep problems, to stress management and coping strategies. It can make referrals to other services to access vital support or social care.

Who is eligible for the service?

The service is for those who fulfil the following criteria:

  • Informal carers, aged over 18
  • Carers must be registered with a GP practice within Guildford and Waverley
  • The person they are supporting has been diagnosed with dementia and is living in their own home

Healthcare professionals can also access the service for specialist advice.

How to access the service?

You can access the service through referral to the Community Co-ordination Centre (01483 362020) or from your GP, health or social care professional, or following a discharge from hospital.
