At the end of your treatment we develop an individulalised ‘end of treatment summary.' The follow-up care you receive will depend on your diagnosis and this will be explained to you. 

You may be offered patient initiated follow-up. This is when you initiate an appointment when you need one, based on your symptoms and circumstances. This will help you to manage your own condition.
