These are our annual staff awards when we publicly recognise those members of staff or a team who’ve been nominated by you as going above and beyond. 

To help recognise our people who shine and make a real difference to you – our patients and their families and carers – we are asking you to tell us about your experience and nominate them for our Patients’ Choice Award.

How to nominate

Anyone can nominate. If you feel you, your family or carer received service or care that made you want to say “thank you” to a team or individual, you can nominate them by completing our online form.

Or you can pick up a hard copy of a nomination form from our hospital receptions. 

Closing date for 2024 nominations: Tuesday 23 July at 11.59pm.

What to include on your nomination form

The nomination form is the only information that our judges will use to assess who receives the awards. For the Patients’ Choice Awards, they will be looking for examples of where colleagues and teams have:

•        Shown dedication above and beyond the call of duty
  
  
•        Had a positive impact on patient experience or patient safety
