Our Acute Frailty Service is dedicated to providing specialised care for elderly individuals living with frailty. We address the individual needs of older adults and offer comprehensive support to enhance well-being, health and independence.

About the condition

Frailty is a medical condition characterised by decreased physiological reserves and increased vulnerability to stressors, leading to heightened risks of adverse health outcomes. It often manifests as diminished strength, endurance, and resilience, resulting in functional decline and susceptibility to injury or illness.

Comprehensive assessment

Our team conducts thorough assessments to identify frailty and associated health issues, allowing us to create personalised care plans tailored to each individual's needs.

Rapid intervention

We prioritise timely intervention to address acute health concerns and prevent further decline in functional status, making sure that patients receive the care they need when they need it most.

Multidisciplinary approach

Our multidisciplinary team comprises geriatricians, nurses, therapists and social workers. They work collaboratively to provide holistic support for the physical, cognitive and psychosocial aspects of frailty.

Holistic support

We focus on more than just medical treatment. Our approach encompasses various aspects of care, including rehabilitation, emotional support and assistance with activities of daily living, to promote overall wellbeing.

Patient-centred care

We prioritise the preferences and goals of our patients, involving them and their families in decision-making processes to make sure that care plans align with their values and wishes.

Preventative strategies

Through proactive management and education, we aim to reduce the risk of hospital readmissions and complications, empowering patients to maintain their health and independence.

Who is eligible for the service?

Patients aged 65 years and above identified as living with frailty.

How to access the service?

We work alongside our colleagues in the urgent and emergency department to identify eligible patients at the point of presentation to our hospital.
