The Learning Disabilities and Autism Team - what do we do?
We aim to make it easier for people to get the right support when they come to hospital, by:
Helping people with a Learning Disability and Autistic people access information in Easy Read.
Co-ordinating the care at any point of their hospital journey.
Helping to prepare for a hospital visit and/or stay.
Providing training to hospital staff and students/trainees.
Helping to make reasonable adjustments in the hospital.
Proving support and guidance to hospital staff.
Helping with transition planning to Adult services in the hospital.
Signposting to other services.
Discharge planning.
